Principal Duties and Responsibilities
Position Summary:Implement and coordinate Bethlehem House Aftercare case management and programming for Bethlehem House Alumni. Provide vision for programming goals, objectives and activities designed to challenge and empower women to overcome hardships and to attain self-sufficiency. Develop community partnerships in faith, education, and health to provide ongoing growth opportunities for Bethlehem House aftercare participants.
Build, enhance and grow relationships with Bethlehem House alumni. Understand and identify the needs of active alumni and culture of our community and provide a pathway to programming with current Bethlehem House residents. This relationship and pathway require mentorship while residents are active at Bethlehem House.
Essential Duties and Responsibilities:
Provide direct case management and home visit services for Bethlehem House active alumni.
Identify short and long-term goals in partnership with alumni. Create working, sustainable, plans with each participant. Identify barriers, networks, and healthy pathways.
Develops and maintains strong working relationships with hospitals, community centers, healthcare providers, medical societies, industry partners, and state/county health departments.
Works collaboratively with aftercare staff to identify group topics for programming needs, and with program staff, to evaluate program activities and developing program expansion projects.
Complete monthly reporting and documentation of all contact and supportive services being provided.
Maintain and foster healthy community relationships that will add value, benefits, and networks to aftercare programming: healthcare, faith communities, churches, community resources, educational institutions, non-profit partners and more.

3-5 years in a health and human services field, or an undergraduate degree in Social Work, or Registered Nurse or Licensed Alcohol Drug Addiction Counselor preferred.
Will deal with individuals in crisis. Requires dealing calmly and effectively with high stress situations. Exposed to a combination of normal office type environments. Must have reliable transportation. Travel is minimal and limited to local meetings.
